Generations Health Care was founded in 2010 by a registered nurse, Alfred Christopher Perez, MSN, RN. Since then, it has grown to multiple establishments currently providing hospice end-of-life care, and visiting physician services in San Antonio, TX and its 13 surrounding counties.
In 2012, Generations Health Care formed a second company Generations Hospice Care to provide a continuum of quality home care by combining their home health services, community/provider services and hospice services. This coming together allows the highly educated and experienced staff of Generations to serve our patients through the entire process of health care.
With the additional strain from launching a second business, Generations recognized the opportunity and used it to create another company to help combat potential struggle. That is when the Company Generations HHC Management Inc. came into fruition. Generations HHC Management, Inc. provides the support needed to ensure our healthcare and hospice agencies run smoothly and efficiently.
In 2013, Generations Visiting Practitioners, LLC. was created granting the ability to admit patients at their local facility. Furthermore, this venture offers yet another way for Generations to visit the patients in the comfort of their own home. This adjoining company further diversified the type of services offered by the Generations’ practice.
Together, the companies each day will fulfill the personal care, case management, skilled nursing, geriatric, and hospice needs of a vast majority of our local residents within our community.

Yes, every GENERATIONS agency meets all required licensing regulations by the Texas Health & Human Services, Regulatory Services Division of Licensing and Credentialing.

Quality of Caregivers
Who will be coming to my home?
GENERATIONS HEALTH CARE, INC. has a team of skilled medical professionals and home health aides who will provide a specific care plan based on your Physician's recommendations, so you can regain a quality level of independence.
GENERATIONS HOSPICE CARE, INC. will assemble an Inter Disciplinary Team which may include a combination of skilled medical professionals and aides who will provide an optimal level of comfort so that you may live each day as fully as possible.
GENERATIONS VISITING PRACTITIONERS, LLC. will provide services consisting of a Nurse Practitioner and Medical Assistant to determine and assess a plan of care in the privacy of your home.
How are the agency's health care professionals screened and can caregivers legally work in the US?
Yes, the GENERATIONS agencies comply with all federal and state regulations regarding work status. All licensed and unlicensed caregivers employed by the GENERATIONS agencies undergo a pre-employment screening and criminal background check.
Supervision and Monitoring of Caregivers
How are the caregivers supervised and monitored to ensure adequate care?
Every GENERATIONS employee is under the direct supervision of a Director of Nursing or Administrator.
GENERATIONS HEALTH CARE, INC. and GENERATIONS HOSPICE CARE, INC. caregivers are required to complete detailed nursing visit notes. A nurse log is also provided to each patient at the time of admission.
When are services available?
GENERATIONS HEALTH CARE, INC. and GENERATIONS HOSPICE CARE, INC. caregivers are available 24 hours a day, 365 days a year. Services are intended to accommodate your schedule and can generally be planned in accordance with your preferences. You can call our office directly during normal business hours Monday through Friday. An on-call nurse is available after-hours and weekends.
GENERATIONS VISITING PRACTITIONERS, LLC. caregivers are available Monday through Friday between 8 am - 5 pm. Services are intended to accommodate your schedule and can generally be planned in accordance with your preferences. You can call our office directly during normal business hours. An on-call nurse is available after-hours and weekends.
Can the agency ensure my confidentiality?
Our GENERATIONS agencies handle patient information in a confidential manner for HIPAA compliance.
What happens if my assigned caregiver is ill or absent?
Our GENERATIONS agencies will arrange a substitute caregiver.
