Step 1: Reach Out
Anyone can initiate the hospice process, a family member, caregiver, doctor, hospital, or even the patient themselves.

Step 2: Free Evaluation
We schedule a free in-home or facility visit, typically within 24 hours. A licensed nurse will:
- Assess the patient’s medical needs
- Explain hospice services
- Determine if hospice eligibility criteria are met
This visit is no pressure, no obligation, just clear guidance and support.
Step 3: Verify Hospice Eligibility
To qualify for hospice, a patient typically:
- Has a terminal illness with a prognosis of 6 months or less (if the illness follows its usual course)
- Is no longer seeking curative treatment
We’ll coordinate with your current doctors or help connect you with a hospice physician.
Step 4: Create a Personalized Plan of Care
Once eligibility is confirmed and the patient/family agrees to begin, our team develops a care plan that may include:
- Pain and symptom management
- Personal care services
- Emotional and spiritual support
- Coordination of medications and supplies
- Regular nursing visits and 24/7 nurse access
We build this plan together based on the patient’s needs, values, and family input.
Step 5: Begin Care
Care can begin right away. Our team will:
- Deliver medical equipment and supplies
- Schedule your first nursing visit
- Introduce your care team (nurse, aide, chaplain, social worker)
- Provide emergency contact info for 24/7 support
Step 6: Ongoing Support
Hospice is not a one-time service, it’s an ongoing relationship. We stay by your side through:
- Regular check-ins
- Adjustments to care as needed
- Emotional support for family
- Bereavement services after loss
