How Hospice Care Works

- Step 1: Getting a Referral. Hospice care typically begins with a referral from a physician, hospital, or case manager. Families can also reach out directly to us to discuss eligibility.
- Step 2: Initial Evaluation. Our Austin-based hospice team will conduct an in-home or facility-based evaluation. We’ll assess the patient’s condition, needs, and preferences at no cost.
- Step 3: Care Plan Development. Once qualified, we create a customized care plan with the patient, family, and referring providers. This plan is updated regularly to reflect changes.
- Step 4: Coordination with Providers. We coordinate with doctors, pharmacists, and other healthcare professionals. We also assist with any durable medical equipment or medication needs.
- Step 5: Ongoing Support. From that point forward, we provide care where the patient lives, whether at home, in assisted living, or in a long-term care facility.
