Step 1: Getting a Referral.
Hospice care typically begins with a referral from a physician, hospital, or case manager. Families can also reach out directly to us to discuss eligibility.
Step 2: Initial Evaluation.

Step 3: Care Plan Development.

We offer nursing care, bathing help, comfort support, and personalized care plans.
How is hospice care paid for?
Most of our patients are covered through Medicare, Medicaid, or private insurance.
How often can someone come out?
We work around your needs and preferences. Visits can be scheduled regularly, and emergency support is always available.
How long does hospice care last?
There is no set time limit. Hospice is available for patients with a life expectancy of six months or less, but many receive care longer depending on their needs and evaluations.
What’s the difference between hospice and home health care?
Home health care focuses on recovery and rehabilitation. Hospice is focused on comfort and quality of life when curative treatment is no longer effective or desired.
