Hospice care typically begins with a referral from a physician, hospital, or case manager. Families can also reach out directly to us to discuss eligibility.

Step 2: Initial Evaluation.
Our Austin-based hospice team will conduct an in-home or facility-based evaluation. We’ll assess the patient’s condition, needs, and preferences at no cost.

Step 3: Care Plan Development.
